About the Role

Auburn Crest Hospice is seeking a compassionate and skilled Registered Nurse to join our interdisciplinary team as a Nurse Case Manager. In this role, you will provide high-quality, patient-centered care to individuals and their families facing the end of life.

Key Responsibilities
  • Coordinate Care: Work with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient and their family.
  • Provide Clinical Expertise: Deliver nursing care that is grounded in evidence-based practice and adheres to established standards of care and hospice guidelines.
  • Communicate Effectively: Collaborate with patients, families, and healthcare professionals to ensure seamless care coordination and address any concerns or questions.
  • Assess and Educate: Conduct thorough assessments of patients and families to identify their needs and provide education on end-of-life care and home safety.
  • Document and Report: Maintain accurate and detailed records of patient interactions, care plans, and outcomes, and report any changes or concerns to the Director of Nursing.
Requirements
  • Education and Licensure: Current Registered Nurse licensure in the state of employment and a minimum of one year of nursing experience, preferably in a Home Health/Hospice setting.
  • Certifications and Skills: Certification in Hospice and Palliative Nursing is a plus, and strong clinical, organizational, and interpersonal skills are required.
  • Physical Demands: Ability to lift up to 20 pounds and occasional travel may be required.
