Job Description
We are currently seeking a Transplant Coordinator II to join our team at Penn Medicine. In this role, you will be responsible for coordinating care for patients with end-stage organ failure referred to the Transplant Program. You will work closely with our multidisciplinary team to ensure that patients receive seamless care throughout the transplant process, from referral to post-transplant care.

Your primary responsibilities will include assessing and evaluating patients and families, developing a care plan in collaboration with the transplant team, creating a written, continuous record for each patient, implementing identified medical and nursing interventions, and evaluating outcomes.

We offer a competitive salary range of $83,000 - $103,000 per year, commensurate with experience.
