About the Role

The Advanced Practice RN Ambulatory will be responsible for delivering high-quality patient care, conducting assessments, developing care plans, and implementing evidence-based interventions. This role requires strong critical thinking skills, effective communication, and the ability to prioritize complex patient needs.

Our compensation package includes a salary range of $100,000 - $120,000 per year, depending on experience, as well as comprehensive benefits, including health insurance, retirement savings plan, paid time off, and ongoing education opportunities.

Responsibilities:

  • Develops and implements individualized care plans that address unique patient needs and preferences.
  • Collaborates with interdisciplinary teams to ensure seamless transitions and optimal patient outcomes.
  • Provides education to patients and families on disease management, self-care, and lifestyle modifications.
  • Participates in quality improvement initiatives and contributes to the development of policies and procedures.
  • Engages in professional development activities to stay current with best practices and advancements in the field.
