Palliative Care Social Worker Job Description

At Cynet Health, we are seeking a skilled Palliative Care Social Worker to join our team. As a Palliative Care Social Worker, you will engage in comprehensive social work services for palliative care patients, supporting them and their families with emotional, social, and financial concerns related to illness and treatment. You will collaborate with interdisciplinary teams to formulate patient care plans, provide education and resources to patients and families about end-of-life care and support systems, and participate in orientation and complete required modules prior to starting. This role requires flexibility and adaptability, with a preference for applicants who can commit to minimal time-off requests and are comfortable with the possibility of limited direct interviews.

Key Responsibilities:
- Engage in comprehensive social work services for palliative care patients
- Support patients and families with emotional, social, and financial concerns related to illness and treatment
- Collaborate with interdisciplinary teams to formulate patient care plans
- Provide education and resources to patients and families about end-of-life care and support systems
- Participate in orientation and complete required modules prior to starting
- Ensure all records and interactions are documented accurately and in a timely manner
- Maintain DC licensure where applicable or ensure Maryland licensure if applicable
- Adapt effectively to evolving healthcare environments and patient needs

Requirements:
- Valid State License required at the time of submission
- Flu Vaccine Required - No Exemptions
- Two supervisor references required
- Local rules accepted at lower rate
- No more than 7 days of RTO per 13-week contract, highly preferred
- Must be okay with non-interview approach
