Palliative Care Social Worker Job Description

At Cynet Health, we are seeking a skilled Palliative Care Social Worker to join our team. As a Palliative Care Social Worker, you will provide clinical social work services to identified patients and families, including both outpatient and inpatient services. Your expertise will be invaluable in offering individual, family, and group counseling regarding the impact of illness and hospitalization. You will serve as a coordinating link between patients, families, hospitals, and community resources to ensure continuity of care. Preferred experience with Cerner is a plus. May require rotating weekend duties if hired on a permanent basis. Complete required orientation and training modules prestart and possibly during orientation.

Key Responsibilities:
  • Provide clinical social work services to identified patients and families.
