Job Title: Clinical Patient Care Services Coordinator

Location: Sussex Campus Hospital

Status: Full Time 72 Hours

Shift: Nights

Job Summary:

The Clinical Patient Care Services Coordinator is a key member of the management team responsible for assisting the Nurse Manager in the daily operations of the nursing unit/department. This role requires direct supervision of staff, rotating shifts and weekends as needed to ensure management presence and visibility.

Responsibilities:

  1. Collect and analyze patient healthcare data to determine patient needs.
  2. Develop and implement individualized plans of care to attain expected outcomes.
  3. Provide direct patient care and coordinate with the healthcare team to ensure continuity of care.
  4. Participate in performance improvement activities and contribute to the development and evaluation of nursing standards and protocols.
  5. Assist in the selection, evaluation, and development of staff members.
  6. Collaborate with the healthcare team to promote a culture of safety, quality, and excellence.
  7. Ensure compliance with regulatory, safety, legal, and ethical standards.
  8. Participate in the development and evaluation of unit standards and protocols.
  9. Contribute to a learning environment that is conducive to healthcare education.
  10. Participate in decision-making and take action based on ethical principles.
  11. Collaborate with the patient, family, and other healthcare providers to provide patient care in a caring environment.
  12. Consider factors related to safety, effectiveness, and cost in planning and delivering patient care.
  13. Provide exceptional service to all internal and external customers.
  14. Promote Service Excellence by identifying and holding staff accountable for core departmental Service Excellence measures.
  15. Maintain compliance with JCAHO standards and all federal and state laws and regulations.
  16. Achieve department/unit specific quality objectives and service excellence objectives.

Required Education, Credential(s) and Experience:

  • Education: Bachelor Degree in Nursing from an accredited nursing program/university.
  • Credential(s): Registered Nurse, Basic Life Support, and related certification within 12 months of hire if not already obtained.
  • Experience: One year in area of related practice; preferred three years in related practice.
