Key Responsibilities

The Case Manager will be responsible for managing a caseload of veteran residents, assisting them in securing permanent housing while adhering to the policies and standards of Axess Family Services.

Primary Duties Include:

  1. Conducting comprehensive assessments of the needs of veteran residents and their households to create tailored support plans focused on housing stability.
  2. Providing a broad range of practical and emotional support services to veteran residents and their families.
  3. Helping veterans develop actionable housing plans with measurable goals and timelines.
  4. Monitoring and adjusting support plans in response to the evolving needs of residents.
  5. Encouraging resident participation in decision-making processes regarding their services.
  6. Coordinating with various agencies to facilitate access to necessary resources and services.
  7. Accompanying residents to appointments with relevant agencies as needed.
  8. Maintaining regular communication with residents to provide ongoing support and interventions.
  9. Ensuring that housing conditions meet established standards of safety and habitability.
  10. Collecting and updating data for the Homeless Management Information System (HMIS).
  11. Providing support to Social Service Workers as required.
  12. Assisting with staffing coverage during on-call shifts.
  13. Engaging in community outreach to connect with homeless and at-risk veterans.
  14. Promoting inclusive practices to ensure all residents are treated fairly and without discrimination.
  15. Adapting to various work environments, whether independently or as part of a team.
  16. Implementing principles of Housing First, Trauma-Informed Care, and Harm Reduction in interactions with veterans.
  17. Participating in training sessions and community events that promote veteran services.
  18. Fostering collaborative relationships with colleagues and other stakeholders.
  19. Ensuring compliance with agency policies, regulations, and safety standards.
  20. Maintaining accurate records and documentation.
  21. Performing additional duties as assigned.
Qualifications

Required:

  • Valid driver's license and ability to meet driving insurability requirements.
  • Successful completion of a criminal background check.

Education:

  • Bachelor's degree or progress towards a degree in social work or a related field is preferred; a high school diploma or equivalent is required.

Experience:

  • Experience with homelessness, domestic violence, substance abuse, and mental health issues is preferred.
  • Experience working with veterans is highly desirable.
