Work Schedule: Flexible, including evenings and weekends; on-call responsibilities
Key ResponsibilitiesThe Case Manager will be responsible for managing a caseload of veteran residents, assisting them in securing permanent housing while adhering to the policies and standards of Axess Family Services.
Primary Duties Include:
- Conducting comprehensive assessments of the needs of veteran residents and their households to create tailored support plans focused on housing stability.
- Providing a broad range of practical and emotional support services to veteran residents and their families.
- Helping veterans develop actionable housing plans with measurable goals and timelines.
- Monitoring and adjusting support plans in response to the evolving needs of residents.
- Encouraging resident participation in decision-making processes regarding their services.
- Coordinating with various agencies to facilitate access to necessary resources and services.
- Accompanying residents to appointments with relevant agencies as needed.
- Maintaining regular communication with residents to provide ongoing support and interventions.
- Ensuring that housing conditions meet established standards of safety and habitability.
- Collecting and updating data for the Homeless Management Information System (HMIS).
- Providing support to Social Service Workers as required.
- Assisting with staffing coverage during on-call shifts.
- Engaging in community outreach to connect with homeless and at-risk veterans.
- Promoting inclusive practices to ensure all residents are treated fairly and without discrimination.
- Adapting to various work environments, whether independently or as part of a team.
- Implementing principles of Housing First, Trauma-Informed Care, and Harm Reduction in interactions with veterans.
- Participating in training sessions and community events that promote veteran services.
- Fostering collaborative relationships with colleagues and other stakeholders.
- Ensuring compliance with agency policies, regulations, and safety standards.
- Maintaining accurate records and documentation.
- Performing additional duties as assigned.
Required:
- Valid driver's license and ability to meet driving insurability requirements.
- Successful completion of a criminal background check.
Education:
- Bachelor's degree or progress towards a degree in social work or a related field is preferred; a high school diploma or equivalent is required.
Experience:
- Experience with homelessness, domestic violence, substance abuse, and mental health issues is preferred.
- Experience working with veterans is highly desirable.
