Key Duties:
  • Facilitate communication between the Director of Nursing and the clinical staff to promote teamwork and efficiency.
  • Ensure compliance with all relevant state and federal regulations to uphold operational standards.
  • Deliver thorough training and education to nursing staff regarding updated protocols and practices.
  • Engage in the recruitment and selection of nursing personnel to build a competent team.
  • Oversee both clinical and administrative tasks, including accurate documentation and record management.
  • Respond to inquiries and referrals with professionalism and empathy.
  • Participate in on-call rotations and provide direct nursing care when necessary.
  • Supervise the care provided by nursing staff, ensuring quality and effectiveness.
  • Work collaboratively with healthcare providers to develop and implement patient care plans.
  • Create, execute, and revise individualized care plans tailored to patient needs.
  • Manage Quality Assurance and utilization review processes, implementing improvements as required.
  • Conduct regular supervisory visits to ensure comprehensive client care coordination.
Qualifications:
  • Valid RN license with a good standing.
  • At least one year of experience in a home health environment.
  • Familiarity with OASIS documentation and relevant clinical software.
  • Demonstrated leadership and management capabilities, with a focus on team motivation.
  • In-depth knowledge of regulations governing home health services.
  • Exceptional communication and interpersonal skills, crucial for effective collaboration.
  • Able to excel in a fast-paced and dynamic work setting.
Work Schedule:
  • This is a full-time role with an 8-hour shift, Monday through Friday.
  • On-call duties may be required as necessary.
