About the Role

At Grand Lake Mental Health Center, we are seeking a highly skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in providing support services to individuals and families, helping them navigate the mental health system and achieve their goals.

Key Responsibilities
  • Conduct non-diagnostic interviews with clients and their families to assess service needs and identify barriers to social, economic, and employment opportunities.
  • Develop and implement care plans that address the unique needs and goals of each client, working collaboratively with other agencies and service providers.
  • Provide crisis intervention services on a 24-hour basis, utilizing your skills and expertise to support clients in times of need.
  • Maintain close communication with the Integrated Team Manager and other clinical and administrative staff to ensure the highest quality of services are delivered.
  • Provide targeted support services, a system of care that is child-centered, with the needs of the child and family dictating the types and mix of services provided.
Qualifications
  • Bachelor's degree in Psychology, Social Work, Sociology, or Criminal Justice, or related field.
  • Minimum of one year of work experience in the mental health field or related social service field, preferred.
  • Current certification as a Behavioral Health Case Manager II.
