- Collaborate with healthcare providers, community resources, and family members to ensure seamless care coordination

- Provide education and support to members and their families on disease management, healthcare options, and community resources
- Act as a liaison between members, healthcare providers, and facilities/agencies to ensure effective communication and care delivery
- Participate in quality improvement initiatives to enhance care delivery and member satisfaction
- Graduate from an accredited school of nursing or a bachelor's degree in a related field
- 4-6 years of experience in care management or a related field
- State licensure and/or compact state licensure as a Registered Nurse (RN) or Nurse Practitioner (NP)
- Resource Utilization Group (RUG) certification (for Superior location)
- Competitive hourly rate: $62.54 per hour
