Job Summary

As a Clinical Pharmacist at St. Elizabeth Healthcare, you will play a vital role in providing pharmaceutical care to patients, focusing on clinical monitoring and therapeutic interventions to improve patient outcomes. You will work closely with the interdisciplinary team to ensure accurate and timely dispensing and distribution of medications.

Key Responsibilities
  • Review and interpret medication orders from licensed prescribers, ensuring they meet the requirements of a complete medication order.
  • Assess drug interactions, allergy alerts, duplication of therapy, and potential therapeutic interactions.
  • Obtain clarification from prescribers for incomplete or potentially suboptimal medication orders.
  • Provide therapeutic alternatives and perform second verification of medication orders for pediatric patients and chemotherapy orders.
  • Direct medication therapy within approved Collaborative Care Agreements and medication management protocols.
  • Document medication therapy assessments and recommendations in patient charts using iVents and progress notes.
  • Supervise compounding, verification of ingredient compatibility, and labeling according to USP standards.
  • Check medications prior to final dispensing for direct patient administration, automated dispensing cabinets, and floorstock dispensing.
  • Use barcode technology to ensure accurate dispensing.
  • Supervise repackaging of medication, verifying and documenting lot numbers, expiration dates, and personnel involved.
  • Provide active direct supervision and mentoring of interns and technicians, monitoring timely completion of tasks and encouraging use of the AIDET model.
  • Detect and report variances in dispensing, identify opportunities for safe medication use, and report suspected adverse drug reactions.
  • Participate in controlled substance accountability, reconciling inventories, and resolving discrepancies.
  • Contribute to the positive overall financial status of the department by encouraging prescriber compliance with formulary restrictions and therapeutic interchanges.
  • Assure formulary adherence, process non-formulary requests, and suggest therapeutic alternatives.
  • Participate in IV to PO conversions and make efficient use of time, handing off work when appropriate to minimize overtime.
  • Provide accurate, adequate, and timely drug information to medical staff, nursing staff, and patients.
  • Serve as a preceptor or provide advice to Pharmacy Residents and Pharmacy Interns.
Requirements
  • Graduate from an ACPE-accredited pharmacy college.
  • Current US pharmacist license in the state where work is being performed.
  • Problem-solving and critical thinking skills.
  • Good communication skills.
  • Familiarity with Current Practice Standards (ASHP).
  • Previous hospital experience.
