We are seeking a highly skilled and compassionate Client Care Coordinator to join our team at Choice Of New Rochelle In. As a Client Care Coordinator, you will play a vital role in providing case management and advocacy services to our clients with mental health or physical health illnesses.
Key Responsibilities
- Provide individualized support and guidance to clients to help them achieve their mental health goals, such as obtaining housing, education, and healthcare services.
- Engage in care activities to empower clients to take control of their lives and make informed decisions about their care.
- Collect and enter data into our case management systems to track client progress and ensure seamless communication among team members.
- Advocate for clients' rights and interests, working closely with government agencies, healthcare providers, and other stakeholders to ensure they receive the support they need.
- Develop and maintain strong relationships with clients, families, and community partners to ensure a comprehensive and coordinated approach to care.
Requirements
- Bachelor's degree in a related field, such as social work, psychology, or healthcare administration.
- Minimum of 2 years of experience in case management, social work, or a related field.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Strong problem-solving and critical thinking skills, with the ability to navigate complex systems and advocate for clients' needs.
- Valid driver's license and reliable transportation.
