The role of the Community Engagement Specialist encompasses:

  • Promoting the organization and enhancing community relationships.
  • Utilizing a profound understanding of the experiences, languages, or cultures of the populations served.
  • Assisting in assessing community needs and aligning services to address those needs.
  • Serving as a liaison between members/caregivers and the organization.
  • Establishing and nurturing effective communication channels.
  • Providing culturally relevant health education, information, support, and counseling to members and caregivers.
  • Participating in the creation, execution, and continuous evaluation of the AFC care plan.
  • Conducting in-person visits with members in appropriate settings.
  • Helping members access additional healthcare and community services, and making referrals to suitable service providers when necessary.
