Thom Child & Family Services is seeking a dedicated and compassionate Early Intervention Social Worker/Clinician to join their team. As a key member of our multidisciplinary team, you will work closely with infants, toddlers, and their families to provide home and community-based early intervention services.
Responsibilities:- Participate in comprehensive assessments of children's developmental status and create individualized intervention plans.
- Provide direct child development services and activities tailored to each child's unique needs and abilities.
- Collaborate with families to identify goals, objectives, and strategies for promoting their child's development.
- Document progress, complete reports, and maintain accurate records.
- Act as a service coordinator, scheduling and implementing intervention plans, and ensuring timely progress monitoring.
- Provide supportive professional assistance to families, empowering them to advocate for their child's needs.
- Master's Degree in Social Work from an accredited institution or equivalent experience.
- Experience working with infants, toddlers, and families is preferred.
- Knowledge of family systems, early childhood development, and evidence-based practices.
- Excellent communication, organizational, and documentation skills.
- Ability to maintain confidentiality and professional boundaries.
- Current driver's license and reliable transportation.
