Job Title: Social Worker Psych PRN Days

At Metrowest Medical Center, we are committed to providing high-quality, comprehensive care to our patients. We are seeking a skilled Social Worker to join our team in a PRN (as-needed) capacity.

Job Summary:

The Social Worker will be responsible for facilitating care along a continuum, ensuring patients receive optimal health, access to care, and appropriate utilization of resources. This role will involve assessing patients for transition needs, identifying and assessing patients at risk for readmission, and conducting complex psycho-social assessments and interventions to promote timely throughput, safe discharge, and prevent avoidable readmissions.

Key Responsibilities:
  • Conduct complex psycho-social transition planning assessments and interventions
  • Assist with adoption, abuse, and neglect cases, including assessment, intervention, and referral to local, state, and/or federal agencies
  • Care coordinate and implement or oversee the implementation of transition plans
  • Lead and/or facilitate multi-disciplinary patient care conferences, including Complex Case Review
  • Make appropriate referrals to other departments and communicate with patients and families about the plan of care
  • Collaborate with physicians, office staff, and ancillary departments to ensure patient education is completed to support post-acute needs
  • Timely complete and concise documentation in the Case Management system
  • Maintenance of accurate patient demographic and insurance information
  • Precept new staff members and act as a resource to all staff
  • Participate in department quality improvement initiatives
Requirements:
  • Master's of Social Work (MSW) degree
  • 2 years of acute hospital experience (preferred)
  • Current licensure, certification, or registration to practice as a Social Worker in the state of practice
