Job Summary
We are seeking a highly skilled Registered Dietitian to join our team at Eddy SeniorCare in Rotterdam, NY. As a key member of our interdisciplinary team, you will work closely with participants and their families to develop personalized care plans that address their nutritional needs. Your expertise will help minimize preventable infections, weight loss, and wounds, while also promoting healthy eating habits and self-management skills.

Key Responsibilities:

  • Conduct comprehensive nutrition assessments and create tailored care plans for each participant.
  • Provide medical nutrition therapy, including diet modification, education, and intervention.
  • Collaborate with the physician and nursing team to ensure seamless care delivery.
  • Train and mentor patient services staff and interns on proper nutrition practices.

Requirements:

  • Credentialed as a Registered Dietitian by the Commission on Dietetic Registration (CDR).
  • Licensed by the State Dietetics Licensing/Credentialing Board, in states where required.
  • At least one year of experience in long-term care, preferred.
