About This Role

We are seeking a highly skilled and compassionate Home Health RN Field Nurse to join our team at Stonerise Home Health Weston. As a key member of our care team, you will play a vital role in delivering high-quality patient care and promoting independence and dignity in the comfort of patients' own homes.

Key Responsibilities
  • Assess and Plan Care

Conduct thorough assessments of patients' physical and emotional needs, developing personalized care plans that address their unique requirements.

Coordinate Care Services

Supervise and direct the delivery of care services, ensuring seamless coordination between healthcare professionals and stakeholders.

Provide Education and Support

Develop and implement patient education plans, empowering patients and their families to manage their health and well-being.

Monitor and Evaluate Care

Regularly evaluate patient progress, revising care plans as needed to ensure optimal outcomes.

Requirements
  • Education and Licensure

Graduate of an accredited school of nursing, with active RN licensure and a minimum of one to two years of recent acute care experience in an institutional setting.

Skills and Qualifications

Strong clinical skills, excellent communication and interpersonal skills, and the ability to work independently and as part of a team.
