Job Summary

We are seeking a highly skilled and experienced Registered Nurse to join our team as a Home Health Registered Nurse - Case Manager. As a key member of our care team, you will be responsible for managing patient care plans, collaborating with healthcare professionals, and providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Develop and implement comprehensive patient care plans in collaboration with the interdisciplinary care team
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for optimal care
  • Collaborate with healthcare professionals, including physicians, therapists, and social workers, to ensure seamless care coordination
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed
  • Monitor patient progress, adjusting care plans as needed to ensure optimal outcomes
  • Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and progress
Requirements
  • RN license in the state of employment
  • Graduate from an accredited school of nursing
  • Minimum one year of RN experience in home health care or a related field
  • Current driver's license and ability to travel to patient locations
  • Strong communication and interpersonal skills, with ability to work effectively in a team environment
  • Ability to self-manage time and prioritize tasks to meet deadlines
  • Proficiency in electronic medical records (EMR) software and other technology systems
