About the Role

We are seeking a highly skilled and experienced Nursing Program Coordinator to join our team at Providence Clinical Network (PCN). This is an exciting opportunity to work in a comprehensive health care organization that serves patients across seven states.

Job Description

The successful candidate will be responsible for coordinating multiple aspects of the heart program, including new patient contact, screening evaluation, and communication throughout the initial phase of treatment. They will also coordinate and work closely with other teams to ensure seamless patient care delivery through the continuum of care.

This role requires strong collaboration and liaison skills, as the coordinator will work with referring providers and hospitals to ensure satisfaction with all components of the program. Additionally, they will maintain effective communication with key clinical and administrative leaders at Providence to ensure that managers and providers are aware of patient progression.

Required Skills and Qualifications
  • Bachelor's Degree in Nursing from an accredited school
  • Oregon Registered Nurse License upon hire
  • National Provider BLS - American Heart Association certification upon hire
  • 5 years of clinical experience in acute care, cardiac ambulatory, and/or cardiac interventions
