We are looking for a dedicated Registered Nurse to fulfill the role of Care Coordinator. This position entails overseeing all systems and services necessary for a structured, patient-focused care team approach, guaranteeing high-quality and cost-effective care for the designated patient demographic. Key responsibilities include managing treatment plans, performing assessments, initiating disease management strategies, and ensuring seamless continuity of care through effective discharge planning and resource utilization.
Key Responsibilities:
- Manage the overall treatment process for patients.
- Conduct thorough assessments to identify patient needs.
- Implement disease management protocols effectively.
- Facilitate continuity of care during discharge planning.
- Collaborate with physicians, nursing staff, and other healthcare professionals to achieve optimal patient outcomes.
Qualifications:
- Graduate from a nationally accredited nursing program.
- Possession of an active RN license in NM or eligibility through reciprocal agreement.
- Current CPR for Healthcare/BLS certification.
- A minimum of 1 year of relevant experience in a clinical setting.
- Bilingual proficiency in English and a Native American language is preferred.
Physical Requirements:
This role involves light physical activity with occasional exertion, including standing and walking.
Working Environment:
May encounter minor hazards such as physical risks, dust, fumes, and noise.
This position is integral to the OP Care Management Services at the University of New Mexico.
