Nurse Care Coordinator

As a skilled Registered Nurse, you will play a vital role in providing comprehensive care coordination and case management services to a diverse panel of patients. Your expertise will be essential in coaching patients to improve their health behavior and attain health-related goals, while working collaboratively with healthcare teams.
Key Responsibilities:- Collaborate with healthcare providers, nurses, social workers, patients, families, and caregivers to ensure comprehensive care.
- Recognize and reward performance in a challenging environment.
- Provide clear direction for success in the role and development for other interested positions.
- Graduate from an accredited school of nursing.
- Active, unrestricted licensure as a Registered Nurse in a state.
- Current state driver's license with the ability to travel to hospitals, skilled nursing facilities, patients' homes, and other care sites as needed.
- Full COVID-19 vaccination is an essential job function; compliance with state booster requirements is necessary.
- Ability to perform all essential job functions with or without reasonable accommodation.
- Certification in Geriatric Nursing or Geriatric Case Management.
- 5+ years of nursing experience, preferably with older patients.
- Proficient in computer use, the Internet, and health information technology.
- Demonstrated excellent communication, interpersonal, and organizational skills with a flexible approach to problem-solving.
- Proven ability to work effectively both independently and as a member of an interdisciplinary team.
