Nurse Practitioner

HealthCare Support is seeking a skilled Nurse Practitioner to join their team in Longmont, CO. As a key member of the healthcare team, you will provide high-quality patient care and support to seniors in Larimer County. Your responsibilities will include conducting comprehensive assessments, managing acute and chronic medical conditions, and ensuring accurate documentation. You will also communicate and collaborate with the interdisciplinary care team to provide patients and caregivers with counseling and education. To be successful in this role, you will need to have an active and unrestricted license in the state of Colorado, a graduate degree in nursing, and a minimum of 1 year of NP experience. Board certification through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center is also required. If you have a passion for gerontology and a strong understanding of advanced illness and end-of-life discussions, this could be the perfect opportunity for you.
