Key Responsibilities- Conduct in-home visits to assess patients' medical histories, review systems, and perform physical examinations.
- Develop and implement personalized care plans to maximize patients' dignity and quality of life.
- Perform Transition Care Management visits to ensure seamless transitions between care settings.
- Order, administer, and analyze diagnostic tests as needed.
- Monitor and operate medical equipment, diagnose health issues, and order medications and treatments.
- Consult with healthcare professionals and families to ensure comprehensive care.
- Train patients and their families to manage and prevent illness and injury.
- Certified Nurse Practitioner (ANCC or AANP) with active, unencumbered licensure.
- Family, Geriatrics, or Adult certification experience.
- Prescriptive authorization.
- Experience providing primary care as a family nurse practitioner.
- Proficiency with medical instruments and equipment.
- Knowledge of computer-based data management programs and medical records.
- Excellent verbal and written communication skills.
- 1+ years of clinical experience in a gerontology, family, internal medicine, or endocrinology setting.
- Home Health care or home visit experience.
- Excellent administrative, organizational, and verbal skills.
- Professional image and behavior.
- Solid communication skills with the geriatric population.
- Ability to work independently and as part of a team.
- Detail-oriented and dependable.
- Ability to lift 30-pound bags and navigate stairs and various dwelling conditions.
