Licensed Practical Nurse

Position Summary: Evaluates, organizes, implements, and reviews the nursing care provided to residents in a skilled nursing environment. Responsible for ensuring seamless care for residents across shifts by delivering direct care and overseeing the contributions of CNAs, LPNs, and support staff. Engages in educational initiatives for residents and their families while upholding the mission of the facility.
Key Responsibilities:
- Conducts assessments of residents upon admission and at least every three months, as well as when health conditions fluctuate. Ensures accurate documentation in accordance with Minimum Data Sets.
- Effectively evaluates and reassesses pain levels, employing suitable pain management strategies and educating residents and families on pain control methods.
- Assesses the health status and nursing requirements of residents; establishes goals and prescribes relevant nursing interventions.
- Records alterations in residents' health status and nursing care in progress notes and care plans.
- Interprets results from waived tests and takes necessary actions based on those results.
- Delivers comprehensive resident care in a manner that prioritizes safety and minimizes the risk of medical errors.
- Promotes and sustains a culture centered on safety and quality.
- Submits documentation in a timely, clear, and concise manner.
- Alerts physicians regarding changes in residents' conditions and ensures appropriate follow-up actions are taken.
- Timely execution of physician orders.
- Provides detailed reports to incoming shifts through participation in walking rounds and appropriate documentation for upcoming needs.
- Addresses any concerns raised by residents or families, following up as necessary.
- Implements individualized care plans based on assessments and goals set by the interdisciplinary care team.
- Treats residents and their families with dignity and respect, recognizing and addressing their psychosocial, cultural, ethnic, and spiritual needs.
- Supervises other healthcare team members as required to fulfill the established care plan.
- Exhibits proficiency in technical and manual skills as per RN scope of practice.
- Acts as an advocate for residents, safeguarding their rights.
- Administers medications in accordance with the seven rights of medication administration, minimizing the risk of errors.
- Demonstrates innovative problem-solving skills and approaches situations with creativity.
- Engages with residents, families, colleagues, and management in a compassionate, knowledgeable, and approachable manner.
- Involves residents and families in the planning of care, treatment, educational needs, and discharge strategies.
- Leads resident care conferences and updates care plans as necessary.
- Serves as a mentor for CNAs and LPNs/LVNs, delegating tasks to enhance the performance of the nursing unit.
- Participates in the orientation of new staff and the evaluation of nursing personnel.
- Adheres to infection prevention and control protocols and supervises support staff in these practices.
- Maintains composure in emergency and high-stress situations, instilling confidence in others.
- Consistently complies with facility policies and procedures.
- Displays a genuine interest in and appreciation for the elderly population.
- Exhibits motivation and independence, effectively organizing workload to ensure all responsibilities are fulfilled.
