Job Summary:

Aveanna Healthcare is seeking a compassionate and skilled Licensed Practical Nurse (LPN/LVN) to join our team. As a LPN/LVN, you will provide high-quality, patient-centered care to our medically fragile children and adults in the comfort of their own homes.

Responsibilities:

  • Implement and coordinate patient care plans in collaboration with our interdisciplinary team.
  • Conduct thorough patient assessments and document care provided in our electronic medical record system.
  • Administer medications, treatments, and therapies as prescribed by our physicians.
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Collaborate with our Case Managers to ensure seamless care transitions and coordination.

Requirements:

  • Current, unrestricted LPN/LVN license in the state of practice.
  • Current CPR certification from the American Red Cross or American Heart Association.
  • One year of experience as a LPN/LVN in a clinical care setting, preferably in home health or pediatrics.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
