We are seeking a highly skilled and compassionate Care Coordinator to join our team at KC CARE Health Center. As a Care Coordinator, you will play a critical role in providing strengths-based Case Management services to individuals living with HIV. Your primary responsibility will be to facilitate and monitor client service acquisition and progress, ensuring successful engagement in comprehensive HIV Care.
Key Responsibilities- Program and Quality Management: Maintain accurate and comprehensive client records, documenting interactions and progress according to program standards. Complete program reports and data requests in a timely and accurate manner.
- Assessment and Care Coordination: Conduct comprehensive assessments and create individualized service plans for clients. Evaluate client eligibility for government-funded programs and provide referrals and follow-up as needed.
- Client Education and Advocacy: Provide education to clients on HIV, medication adherence, and risk reduction strategies. Educate clients on the process for submitting insurance and co-pay documentation and work closely with clients and third-parties to maintain insurance coverage.
- Education: Bachelor's degree in Social Work, Nursing, or related field, or 2 years' experience in Case Management type services.
- Experience: At least 2 years' experience in HIV Case Management or related field. Active participation in at least one Clinic committee or Ryan White Planning Council Subcommittee.
- Certifications: Obtain and maintain state-issued rapid HIV/HCV testing method certification for provision of counseling, testing, and referral (CTR) services.
The physical demands of this role include working in a healthcare facility with potential exposure to virus, disease, and infection from patients. May experience traumatic situations, including psychiatric traumatized and deceased patients. Travel may be expected to community meetings, client homes, or other agencies.
