Job Summary:

As a Care Coordinator with Moms In Motion, you will play a vital role in supporting families in the Chesapeake area. Your primary responsibility will be to ensure the safety, satisfaction, health, and welfare of our clients while they live independently in the community.

Key Responsibilities:

• Develop and implement personalized care plans for clients

• Coordinate with healthcare providers, social services, and other stakeholders to ensure comprehensive support

• Conduct regular home visits to assess client needs and provide guidance

• Maintain accurate records and reports to track client progress

Requirements:

• Associate's degree or higher in a human services field, or bachelor's degree in a non-human services field with relevant experience

• 2-3 years of experience in a related field, with a focus on case management or social work

• Excellent communication and interpersonal skills

• Ability to work independently and as part of a team
