The University of Virginia is seeking a highly skilled Clinical Pharmacist to join our team in the Solid Organ Transplant department. As a key member of our transplant pharmacy team, you will provide specialized patient-centered clinical services to the inpatient solid organ transplant population and the outpatient transplant clinic/infusion center.
Key Responsibilities- Provide pharmaceutical care to patients to achieve optimal therapeutic outcomes
- Promote cost-effective drug therapy
- Promote a cooperative work environment by utilizing communication skills, interpersonal relationships, and team building
- Assist with verification and dispensing of medication orders
- Provide assistance in educating other healthcare professionals in regards to pharmacotherapy
- Participate in multidisciplinary and cross-service drug therapy management plans
- Assist in developing and implementing treatment protocols to improve patient care
- Bachelor's in Pharmacy or Pharm. D Degree required
- PGY-1 residency required or equivalent hospital experience
- PGY-2 residency preferred for specialty areas
- Commonwealth of Virginia State Board of Pharmacy license required
- American Heart Association (AHA) Health Care Provider BLS certification required
The job requires sitting for prolonged periods, repetitive motion, and the ability to lift/push/pull < 20lbs. May be exposed to fumes, chemicals, blood/body fluids, and infectious disease.
