Salary and Benefits

The estimated salary for this position is $120,000 per year, depending on experience and location. Job Responsibilities

This role requires 2 years of experience within the last 5 years in chronic disease management/long-term care, preferably in hospice background. You will provide ongoing palliative care and hospice services, collaborating with a team of like-minded clinicians and teammates. As an active member of the medical community in your territory, you will work towards alleviating burdensome symptoms and aligning medical treatments with patients' goals and values.

Qualifications and Requirements
  • A heart to serve patients and families and a passion for providing the best possible care.
  • Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
  • Ability to sit, stand, bend, move intermittently and lift at least 25lbs.
