Job Summary:
Evaluate the clinical needs of newly referred and hospitalized hospice patients. Identify and inform patient and family about appropriate resources. Work with the patient/caregiver and appropriate providers to establish patient/caregiver care goals and identify and implement interventions.
Key Responsibilities:
- Evaluate patient clinical needs and develop a plan of care
- Communicate effectively with patients, families, and healthcare providers
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Identify and implement interventions to meet patient care goals
Requirements:
- Currently licensed as an RN in North Carolina
- Minimum 3-5 years of experience in an acute or home health setting
- Bachelor's degree in nursing and Hospice experience preferred
- RN Certification in Hospice and Palliative Care preferred
