Hospital Liaison Job Description

The Hospital Liaison is a critical role at The Orchards of Michigan, responsible for coordinating all offered services for patients in a facility setting. This involves completing part or all of the admission process for patients, partnering with the admission and marketing team to develop and maintain referral source relationships for assigned facilities, and ensuring a smooth discharge process.

Key Responsibilities:

  • Collect patient information during the referral process
  • Accept patients on behalf of the facility after clinical and financial review
  • Communicate with and educate referral sources regarding services via written and telephone contact
  • Work with hospital discharge planners to facilitate a smooth discharge process
  • Provide assistance to hospital and assisted living discharge planners to coordinate discharge, including contacting families, setting up transportation, and assisting with discharge instructions
  • Facilitate nurse-to-nurse communication between facility nursing staff and hospital/assisted living nursing staff to ensure continuity of care
  • Provide support and review Emergency Department admissions as they present in the Emergency Department
  • Meet with hospital staff on a monthly basis to review the number of cases sent to the facility, discuss the process, and modify any necessary areas
  • Review reasons for readmissions with the hospital on a monthly basis
  • Provide training and education to community groups as needed
  • Serve as a liaison between referral sources, patients, and families
  • Prepare reports for the leadership team as requested
  • Admit patients to the facility service, completing accurate documentation, consents, and assessments as applicable
  • Adhere to work schedules in completing and performing assigned tasks
  • Apply the policies and procedures of the agency and the rules and regulations of Federal and State regulatory agencies in performing the referral and intake processes
  • Participate in agency quality improvement programs
  • Effectively assist the patient, physician, and/or discharging facility to establish an effective care plan prior to the patient's transfer to another setting
  • Meet or exceed established productivity standards
  • Recommend care alternatives, which may include other community resources, when appropriate
  • Evaluate patients for admission to services
  • Advocate on behalf of patients to ensure receipt of entitled benefits
  • Participate and assist in the referral process to ancillary services
  • Adhere to work schedules in completing and performing assigned tasks
  • Follow up on referrals that have not been admitted to care
  • Partner in the development of marketing action plans for assigned facilities
  • Participate in growth meetings and appropriate training sessions
  • Update referral resources on the status of patients currently receiving service or recently discharged and the patient's response to services received
  • Keep current with changes and trends in the market
  • Demonstrate regular communication with referral sources to establish working relationships
  • Ensure growth and profitability of the company through the responsible use of company resources
  • Accept other assignments as appropriate
  • Adhere to established personnel policies
