Job Summary:

  • The Clinical Manager plays a crucial role in ensuring the delivery of high-quality patient care while promoting efficient operations within our home health organization.

  • This position requires a skilled Registered Nurse who can develop, implement, and analyze clinical operations, focusing on both quality assurance and operational initiatives.

  • Key responsibilities include conducting assessments and reassessments of patients, updating care plans, interpreting patient needs, and ensuring adherence to company policies and procedures.

  • You will also manage caregiver assignments, oversee care delivery, receive case referrals, and review patient information to determine home health or hospice needs.

  • In addition, you will instruct and guide clinicians, assist in establishing therapeutic goals, and monitor cases to ensure compliance with regulations.

Requirements:

  • Graduate of an accredited School of Nursing

  • Current state license as a Registered Nurse

  • Proof of current CPR certification

  • Valid driver's license, auto insurance, and reliable transportation

  • Two years of experience as a Registered Nurse, with at least one year of management experience in a home care, hospice, or equivalent environment

  • Home health experience is required; management and people leadership experience is also necessary

  • OASIS experience and certification (HCS-O, COQS, and/or COS-C) are preferred

  • Homecare Homebase (HCHB) experience and CMS PDGM billing knowledge are also preferred
