Job Summary:
The Clinical Manager plays a crucial role in ensuring the delivery of high-quality patient care while promoting efficient operations within our home health organization.
This position requires a skilled Registered Nurse who can develop, implement, and analyze clinical operations, focusing on both quality assurance and operational initiatives.
Key responsibilities include conducting assessments and reassessments of patients, updating care plans, interpreting patient needs, and ensuring adherence to company policies and procedures.
You will also manage caregiver assignments, oversee care delivery, receive case referrals, and review patient information to determine home health or hospice needs.
In addition, you will instruct and guide clinicians, assist in establishing therapeutic goals, and monitor cases to ensure compliance with regulations.
Requirements:
Graduate of an accredited School of Nursing
Current state license as a Registered Nurse
Proof of current CPR certification
Valid driver's license, auto insurance, and reliable transportation
Two years of experience as a Registered Nurse, with at least one year of management experience in a home care, hospice, or equivalent environment
Home health experience is required; management and people leadership experience is also necessary
OASIS experience and certification (HCS-O, COQS, and/or COS-C) are preferred
Homecare Homebase (HCHB) experience and CMS PDGM billing knowledge are also preferred
