Job Summary

This position requires an experienced Registered Nurse to coordinate post-acute services in the home setting. The successful candidate will manage daily caseloads, provide direct patient care, and supervise LPNs and HHAs. They will also collaborate with a multidisciplinary team to ensure high-quality patient outcomes.

Essential Functions:

  1. Manage daily caseloads, ensuring timely care and documentation to meet patient needs and goals.
  2. Collect and analyze data from patient assessments, telephonic contacts, and EMR information to identify issues and develop effective resolutions.
  3. Prioritize multi-issue situations based on patient health and welfare, staff safety, and service impact.
  4. Develop and implement plans of care that support quality patient outcomes and satisfaction.
  5. Supervise LPNs and HHAs to promote healthy outcomes and address any barriers to resolution.
  6. Evaluate progress towards patient-centered goals.
