Healthcare Team Coordinator

As a Home Health Care Coordinator, you will play a critical role in delivering high-quality patient care and coordination with multidisciplinary teams to achieve optimal outcomes. This role is ideal for an individual who is passionate about making a difference in the lives of patients and their families.
Your primary responsibilities will include assessing patients' needs, developing and implementing personalized care plans, and coordinating care with healthcare professionals to achieve optimal outcomes. You will also be responsible for communicating effectively with patients, families, and healthcare teams to ensure seamless care delivery.
Key Responsibilities:

Requirements:
- Graduate of an accredited or approved school of nursing with a degree in AD, Diploma, or BSN program.
- Current license to practice professional nursing in the state where providing care (NC/SC). CPR certification is required.
- Minimum one year RN experience and a total of two years clinical experience is necessary. Supplemental experience may include experience as LPN, CNA, military medic, EMT, or related experience. Home health experience is preferred.
- Ability to transfer and/or maneuver objects weighing at least 50 pounds in the assessment and implementation of patient care.
- Able to communicate and be literate in the English language.
