Job Summary

We are seeking a skilled and compassionate PRN Home Health RN to join our team at HCA Virginia Healthcare at Home. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.

Responsibilities
  • Assess patients' physical, psychosocial, and environmental needs, identifying areas for improvement and developing personalized care plans.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Supervise and provide clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services.
  • Participate in on-call rotation and emergency call, providing timely and effective support to patients and families.
Requirements
  • Basic Cardiac Life Support certification within 30 days of employment start date.
  • Registered Nurse licensure and Associate Degree or Registered Nurse Diploma.
  • 1 year of experience in home health or a related field.
