We deliver a comprehensive suite of evidence-based rehabilitative services, including mental health and substance abuse treatment, housing assistance, vocational training, healthcare, education, and creative arts therapies.
Role Summary
As a Case Manager, you will play a crucial role in delivering supportive services to formerly homeless individuals diagnosed with serious mental health issues and/or substance use disorders, residing in independent living situations, whether in a single site or scattered throughout the community.
Key Responsibilities:
- Facilitate case management services, including assistance with benefits, housing applications, and connections to primary and behavioral healthcare services in the community.
- Ensure timely and accurate documentation for all client interactions in compliance with agency policies and relevant regulations.
- Assist in preparing for program audits.
- Provide crisis intervention when necessary.
- Maintain compliance with all operational requirements, including managing admissions and discharges, documentation, and incident reporting.
- Collaborate with other departments and external agencies.
- Engage with clients, families, and staff in a respectful and culturally sensitive manner.
- Conduct community visits to client residences, utilizing stairs and public transportation as needed.
- Report serious incidents and sensitive situations to supervisors immediately, completing necessary incident reports as per policy.
- Participate in staff meetings, supervision, and training sessions as required.
- Perform additional duties as assigned.
Essential Skills and Experience:
- Bachelor's Degree is mandatory.
- Experience in mental health, crisis intervention, and trauma-informed care is essential.
- Strong organizational and time management skills.
- Ability to work collaboratively in a team environment, demonstrating resilience, flexibility, and openness to feedback.
- Willingness to obtain CPR/First Aid/AED/Naloxone certification.
- Familiarity with AWARDS software is preferred.
- Experience working with Veterans is an advantage.
