Job Title: Registered Nurse - Hospice Care

We are seeking a compassionate and skilled Registered Nurse to join our team at St. Croix Hospice. As a Registered Nurse - Hospice Care, you will play a vital role in providing high-quality patient care and support to individuals and their families in their homes and communities.

Job Summary:

The Registered Nurse - Hospice Care will be responsible for organizing and directing hospice care, with a focus on community health education and experience. You will work closely with patients, families, and healthcare teams to develop and implement individualized care plans that meet the unique needs of each patient.

Key Responsibilities:
  • Provide comprehensive assessments and evaluations of patients' physical and emotional needs
  • Develop and implement individualized care plans that address patients' medical, emotional, and spiritual needs
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination
  • Provide education and support to patients and families on disease management, symptom control, and end-of-life care
  • Document patient progress and outcomes in a timely and accurate manner
  • Participate in interdisciplinary team meetings to discuss patient care and develop care plans
Requirements:
  • Graduate of an accredited school of nursing with a current RN license
  • 1-2 years of recent acute care experience in an institutional setting and 1-2 years of recent experience in home care
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist
  • Prolonged or considerable walking or standing, able to lift, position, and/or transfer patients, and able to lift supplies and equipment
