We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team at Providence. As an LPN, you will play a vital role in providing high-quality patient care in the comfort of their own homes.
Key Responsibilities- Provide comprehensive nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct thorough assessments of patients' physical and emotional needs, and document findings in a timely and accurate manner.
- Administer medications and treatments as prescribed by physicians, and monitor patients for adverse reactions.
- Provide education and support to patients and their families on disease management, medication use, and self-care techniques.
- Participate in ongoing quality improvement initiatives to ensure the highest level of patient care and satisfaction.
- Graduate of an accredited LPN/LVN school of nursing.
- National Provider BLS - American Heart Association within 30 days of hire.
- Oregon Licensed Practical Nurse upon hire.
- Valid driver's license and reliable transportation.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Experience in home health, hospice, or public health.
- Knowledge of disease management and treatment options.
- Ability to work with diverse patient populations and families.
