Job Summary

We are seeking a highly skilled and dedicated Physical Therapist to join our team at WMCHealth. As a Physical Therapist, you will play a vital role in the assessment and treatment of clients needing physical therapy services in the home setting.

Key Responsibilities
  • Clinical Skills and Knowledge
    • Perform complete patient assessment, including patient/family learning needs.
    • Identify key physical therapy diagnosis from the assessment data to address all pathophysiologic and psychosocial patient needs.
    • Prioritize physical therapy diagnoses and establish a data-driven plan of care.
    • Implement individualized plan of care and discharge plan, including coordination with other disciplines.
    • Evaluate outcomes of plan-of-care and alter plan as needed.
    • Research and assist in the development of patient education tools.
    • Document patients' progress in a timely manner.
    • Recognize and intervene in situations related to patient safety.
  • Communication
    • Advocate for patient well-being, including religious/culture preferences, privacy, etc.
    • Communicate patient data in clear, concise, and timely manner, both in written and verbal form.
    • Individualize communication based on assessment of patient/family.
    • Confer with case manager and other interdisciplinary team members per policy.
  • Management of Care
    • Delegate and supervise patient care appropriately.
    • Assist rehabilitation manager in performance appraisals.
    • Cover for rehabilitation manager in their absence.
    • Participate in agency-based QI projects.
    • Initiate an agency-specific project related to clinical care of patients.
  • Professional Development
    • Function and uphold the professional standards of the physical therapy role via evidence-based practice, interactions, and appearance.
    • Identify potential areas of therapy research.
    • Help participate in orientation and presenting of staff as needed.
    • Accept accountability for own practice.
    • Research and assist in the development of staff education tools.
    • Identify potential learning needs of staff.
Requirements
  • Experience
    • Minimum of two years of experience in an acute care setting.
  • Education
    • Graduation from an accredited school of physical therapy, minimum Bachelor's Degree.
  • Licenses/Certifications
    • Current license to practice physical therapy in New York.
  • Other
    • If applicable, the individual performing this job may reasonably anticipate coming into contact with human blood and other potentially infectious materials.
    • Individuals in this position are required to exercise universal precautions, use personal protective equipment and devices, and learn the policies concerning infection control.
    • New York State driver's license.
    • Mental and physical health sufficient to meet demands of the position.
    • Ability to work both independently and with the healthcare team.
