Responsibilities- Conduct diagnostic evaluations of patients, set realistic goals, and re-evaluate progress regularly.
- Meet productivity standards for evaluations and treatments.
- Review patient background information and prioritize clinical assessments.
- Interpret evaluation results and make sound clinical recommendations.
- Formulate functional short- and long-term goals that are patient-specific and consistent with evaluation results.
- Present results to patients, families, caregivers, and treatment teams.
- Provide individualized evidence-based treatment that is consistent with evaluation results and physician prescriptions.
- Meet productivity standards for treatments provided.
- Select and utilize safe and effective treatment procedures and techniques.
- Observe patient response to treatment and modify methods as needed.
- Provide patients with individualized home treatment activities and instructions.
- Coordinate services with other healthcare providers.
- Adequately plan and prepare for patient discharge.
- Identify discharge criteria, methods for achieving them, and possible obstacles.
- Discharge patients from care with appropriate referrals and recommendations.
- Communicate with patients and families in a manner that facilitates their understanding of treatment and achievement of positive outcomes.
- Completion of clinical affiliations as outlined by the American Physical Therapy Association.
- Graduation from an accredited program with a Bachelor's, Master's, or Doctoral degree in Physical Therapy.
- New York State registration and license to practice as a Physical Therapist.
- Ability to form effective working relationships with hospital personnel, multidisciplinary teams, patients, and families.
