We are seeking a highly skilled Clinical Operations Manager, RN to join our team at Island Home Care in Key West. This is a full-time position that requires the ability to work in our Key West office and have an active RN License in Florida.

About the Role

The Clinical Operations Manager, RN will be responsible for supervising and coordinating clinical services to ensure high-quality care for home health patients. They will lead an interdisciplinary team of staff to develop and implement individualized patient plans of care, while ensuring compliance with state and federal regulations.

In this role, you will:

  • Coordinate referrals and ensure timely patient evaluations by clinician and/or therapist assignments
  • Ensure patient needs are continually assessed and care rendered is individualized, meets eligibility criteria, and aligns with physician orders
  • Oversee development, implementation, and updates to patient plans of care
  • Manage and document phone calls from physicians, clinicians, patients, and referral sources
  • Review assessments and plans of care daily, consult with clinicians, and make recommendations as needed

You will also be responsible for:

  • Coordinating all aspects of care with disciplines, physicians, durable medical equipment providers, caregivers/family members, transferring facilities, and other healthcare providers
  • FOLLOWING-UP on lab and other clinical diagnostic tests, physician contact, and significant changes in patient condition
  • Scheduling, preparing for, facilitating, and documenting case conferences/SOC reports
  • Assisting clinicians in coordinating patient transfers and discharges
  • Receiving report from field clinicians prior to scheduled days off on patient status and ongoing needs

Requirements

To be considered for this role, you must have:

  • A current RN licensure in the state of Florida
  • Current CPR certification
  • A valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation

Please note that this position requires regular travel to different locations within Key West.

Estimated Salary: $85,000 - $110,000 per year
