About the Role

We are seeking a skilled Ambulatory Care Nurse Specialist to join our team at Cypress Healthcare Partners. This is a challenging and rewarding opportunity for a dedicated and passionate Registered Nurse who wants to make a meaningful impact in the lives of our patients.

Key Responsibilities
  • Support physicians and advanced practice providers in developing, executing, and evaluating patient care plans.
  • Closely monitor patients' response to care plans, including medication adherence and at-home self-care as applicable.
  • Coordinate pre-visit planning activities to ensure patients have successful treatment and follow-up.
  • Lead clinical daily huddles with medical assistants and licensed vocational nurses.
  • Provide reinforcing education and coaching to patients through end-of-visit consultations.
Requirements

To be considered for this role, you should have:

  • A minimum of 3 years of practice as an RN with at least 1 year of Primary Care experience.
  • Experience with electronic medical record documentation.
  • Working knowledge of quality standards and evidence-based practice.
  • California licensed Registered Nurse.
  • Strong letters of recommendations (2).
Salary

The estimated salary for this role is $85,000 - $110,000 per year, depending on experience.
