Job Title: Home Health Registered Nurse RN Full Time

Job Summary:

Aveanna Healthcare is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse RN Full Time. As a Home Health Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence, dignity, and comfort.

Responsibilities:

  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Provide skilled nursing care, including wound care, infusion therapy, and catheter care.
  • Conduct thorough assessments, identify patient needs, and develop effective care plans.
  • Collaborate with multidisciplinary teams to ensure seamless care transitions and coordination.
  • Provide education and support to patients and families on disease management, medication, and self-care.
  • Document patient progress, outcomes, and care plans in accordance with regulatory requirements.

Requirements:

  • Active RN license in the state of application.
  • Valid CPR certification.
  • Medicare Skilled Nursing experience preferred.
  • Basic understanding of Oasis.
  • 1-year RN experience in a healthcare setting.
