Job Summary:

Aveanna Healthcare is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse. As a Home Health Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, working collaboratively with our interdisciplinary team to develop and implement personalized care plans.

Responsibilities:

  • Develop and implement individualized care plans in accordance with physician orders and patient needs.
  • Provide skilled nursing care, including wound care, infusions, catheter care, and PICC line dressing changes.
  • Educate patients and their families on disease management, medication adherence, and self-care techniques.
  • Collaborate with primary caregivers to ensure optimal functioning and provide training as needed.
  • Coordinate care with the patient's physician and other healthcare providers to ensure seamless transitions and continuity of care.

Requirements:

  • Active RN license in the state of application.
  • Valid CPR certification.
  • Medicare Skilled Nursing experience preferred.
  • Basic understanding of OASIS and Homecare Homebase experience a plus.
