As a skilled Home Health Registered Nurse RN, you will provide high-quality, patient-centered care to individuals in their homes. This role requires strong clinical skills, effective communication, and a passion for delivering exceptional patient outcomes.

Responsibilities:

  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Conduct thorough assessments, identify patient needs, and provide education on disease management, medication, and self-care.
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
  • Provide hands-on care, including wound management, infusion therapy, and catheter care, as needed.
  • Document patient progress, communicate with healthcare teams, and maintain accurate records.

Requirements:

  • Active RN license in the state of application.
  • Valid CPR certification.

Preferred Qualifications:

  • Medicare Skilled Nursing experience.
  • Basic understanding of OASIS.
  • 1-year RN experience in a healthcare setting.
  • Homecare Homebase experience a plus.
