POSITION SUMMARY

The Clinical Pharmacist plays a vital role in delivering high-quality pharmaceutical services to patients. This position collaborates closely with the clinical team, including the director, healthcare providers, and other staff, to enhance pharmacy operations at Heartland Community Health Center.

KEY RESPONSIBILITIES
  • Engage with patients and staff to facilitate access to medications.
  • Review and verify prescriptions for accuracy, determining necessary ingredients and formulations.
  • Fill prescriptions safely and efficiently, adhering to all relevant laws and regulations.
  • Identify and resolve medication-related issues, providing evidence-based recommendations for therapy.
  • Coordinate medication therapy management (MTM) with other clinic personnel.
  • Support quality improvement initiatives by monitoring patient medication adherence and appropriateness.
  • Educate patients and caregivers on medication usage, disease management, and safety measures.
  • Provide drug information and education to patients and healthcare team members.
  • Administer immunizations as required.
  • Offer telehealth services to patients when appropriate.
  • Supervise pharmacy technicians and interns.
  • Assist the Director of Pharmacy in maintaining accreditation standards.
  • Refer patients to external healthcare services as necessary.
  • Manage patients under collaborative practice agreements for chronic conditions.
  • Support providers in managing anticoagulation therapy.
  • Handle financial transactions accurately and maintain medication orders.
  • Respond to inquiries and maintain compliance with organizational policies.
  • Ensure understanding of the 340B program and assist with compliance.
  • Prepare prescriptions for delivery or mailing as needed.
  • Perform additional duties as assigned.
QUALIFICATIONS
  • Preferred experience in FQHC/340B pharmacy settings.
  • Current Kansas Pharmacist's License required.
  • Continuing Education must be maintained as mandated.
  • Ambulatory Care Residency or BCACP Certification preferred.
  • Minimum of 2 years of experience in clinical patient care management preferred.

  • Proficient in Microsoft Office, pharmacy management systems, and EHR software.
  • Ability to handle emergencies calmly and effectively.
  • Maintain confidentiality in accordance with HIPAA regulations.
  • Strong written and verbal communication skills.
  • Ability to perform basic mathematical calculations.
WORK ENVIRONMENT
  • Regular movement and manual dexterity required.
  • Occasional lifting of items up to 25 pounds.
  • Frequent computer use.
  • Good lighting and comfortable working conditions are maintained.
  • Close physical proximity to staff and patients is common.
PHYSICAL DEMANDS
  • Visual acuity to operate equipment effectively.
  • Ability to read, write, and communicate in English.
  • Effective hearing for communication purposes.
