Core Measure Data Analyst

We are seeking a highly skilled Core Measure Data Abstractor to join our team at Cayuga Health System. As a key member of our healthcare team, you will be responsible for collecting and abstracting data from patient medical records, ensuring high levels of abstraction accuracy for assigned accounts via validation activities.
Key Responsibilities- Data Collection and Abstraction: Collect and abstract data from patient medical records, focusing on disease-specific care, diagnoses, treatments, procedures, and outcomes.
- Data Accuracy and Completeness: Ensure the accuracy and completeness of abstracted data by cross-referencing information from multiple sources within a patient's medical record.
- Clinical Data Abstraction Guidelines: Adhere to specific clinical data abstraction guidelines and standards, such as those set by the Centers for Medicare and Medicaid Services (CMS) and The Joint Commission (TJC).
- Data Entry and Maintenance: Enter abstracted data into a database or registry, using specialized software, and maintain and update the data as necessary.
- Quality Assurance: Participate in quality assurance processes to ensure data integrity, including regular audits of the data or the abstraction process.
- Education and Experience: High School Diploma or Equivalent; experience abstracting Core Measures within the last 2 years is required.
- Skills and Qualifications: Familiarity with medical records, billing/documentation practices, and standard healthcare quality concepts; ability to work independently; relies on experience and judgment to plan/accomplish goals; maintains a strict level of confidentiality in all aspects of work.
- Education and Experience: BSN, LPN, or RN preferred.
- Location and Travel: Hybrid / Remote; travel required within the various CHS locations.
- Pay Disclosure: $58,458.40 - $75,000.
