Job Description

We are seeking a highly skilled and compassionate Case Manager to join our team at CONCERN. As a Case Manager, you will play a vital role in providing support and guidance to individuals and families in need. Your primary responsibility will be to work closely with clinical supervisors to manage caseloads, process intakes and discharges, and schedule evaluations. You will also be responsible for maintaining accurate records and ensuring compliance with HIPAA regulations.

Key Responsibilities:

  • Process incoming clients for assigned BHS programs
  • Work closely with clinical supervisors to manage caseloads and admissions
  • Schedule psychological evaluations and facilitate interagency meetings
  • Prepare and maintain client files and participate in agency meetings
  • Ensure compliance with HIPAA regulations and maintain confidentiality

Requirements:

  • Bachelor's degree in human services or related field
  • Two years of experience in human services
  • Valid driver's license and ability to travel
  • Ability to work independently and as part of a team
  • Excellent organizational, interpersonal, and communication skills
