Summary:

As a vital member of the Community Based Care Float Pool, the individual will provide support across various Catholic Health facilities' Long Term Care settings to address staffing shortages and absences. The Practical Nurse is responsible for delivering nursing care to assigned residents under the supervision of a registered nurse. This role includes coordinating and assisting the certified nursing assistants while actively engaging in the direct care of residents, adhering to the outlined care plans.

Key Responsibilities:

EDUCATION:

  • High School diploma or GED
  • Graduation from an accredited practical nursing program
  • Current registration as a Licensed Practical Nurse in New York State

EXPERIENCE:

  • A minimum of three (3) months of experience in long-term care is required

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Excellent written and verbal communication skills

WORKING CONDITIONS:

  • Physically demanding role involving lifting and moving residents, with extended periods on foot

ENVIRONMENT:

  • Potential exposure to challenging conditions, including odors, communicable diseases, and risks associated with resident care and equipment
