Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in a Continuous Care role. As an LPN, you will provide high-quality patient care and support to patients and their families in times of crisis.

Key Responsibilities
  • Under the direction of the RN case manager, provide continuous bedside clinical observations, prescribed care, physical, and emotional support to patients as directed in the plan of care.
  • Accept and process physician orders to maximize patient comfort in collaboration with the RN.
Requirements

To be successful in this role, you must be a graduate of an approved nursing program and hold a current Florida LPN license. A minimum of one year of experience in an acute care setting, providing direct patient care, is also required. Previous experience as a hospice LPN is preferred.
